Request Form

1. Fill out a Request Form. If you have questions, contact Aishah Pacheco - Event Manager at:

2. The event manager will contact you about your date. A contract will be mailed 2 to 3 months prior to your event.
3. Sign and return both copies of contract along with:
     1. Risk Management Form
     2. Ticket Set-Up Form
     3. Proof of Insurance (excluding campus groups)
     4. 501(c)3 status if applicable
     5. PeopleSoft information sheet if campus group
     6. Deposit of 25% rental fee
4. Call Aishah Pacheco to set up pre-production meeting to determine technical and box office costs.
5. Send in remainder of rental fee and technical charges 30 days before event date.
6. Have a successful, well-planned event.
7. You will receive final settlement invoice or payment within 30 days of the event.
